Billing Information

Quality of care is a patient's first priority when seeking medical attention. This is our priority too. You will also find that our fees are comparable to other medical offices in the area and are significantly lower than those charged by hospital emergency rooms.

San Benito Health Foundation accepts most forms of health insurance, including Medi-Cal, Medicare, Dental-Cal, and many public and private insurance plans. If you have health insurance, please bring your insurance card to each appointment.

If you or your family members do not have health insurance, we can help you apply for coverage, including new insurances available under Covered California.

We also offer sliding fee scale discounts depending on your family size and income. If you qualify for a sliding fee scale discount, it will apply to a variety of services. We also have special discount programs for eligible patients in family planning, prenatal care, well-child care, immunizations, school health physicals and mental health.

Depending on eligibility and billing requirements, payment is expected at the time services are rendered; all co-payments must be paid before services are rendered. You will be responsible for any balance of fees not covered by a third-party payer.

If you need to cancel a visit, you must call 24 hours in advance. Failure to cancel an appointment will result in a "NO SHOW" $20.00 fee.

Sliding Fee Scale Eligibility

All individuals that choose to receive health care services at San Benito Health Foundation are required to first register as patients and renew as patients annually. During the registration and annual renewal process, patients are interviewed to determine method of payment and eligibility for various benefit health plans and programs, including eligibility for Sliding Fee Scale discount.

In accordance with the Federal Poverty Guidelines, SBHF’s sliding fee scale is based upon family size and income. SBHF does not discriminate against any individual receiving health services because of their inability to pay for services. If you have questions about this policy or wish to inquire about your eligibility to qualify for various benefit health plans and programs or sliding fee scale discounts, please speak with a receptionist at the front desk.

Estimate of Charges

All patients have a right to a good faith estimate of expected charges.
